Referrals from other health care providers

The state of Minnesota requires clients to pay sales tax on the services I provide, unless we have a referral on file from a licensed health care professional, stating that the massage/craniosacral work is for ongoing treatment of illness, injury or disease. These letters are short and simple, and don't need to name any specific diagnoses. Here are just a few examples of these licensed professionals, part of the longer list on this site:
· Chiropractor
· Dietician or Nutritionist
· Licensed Professional Clinical Counselor (LPCC)
· Licensed Marriage and Family Therapist (LMFT)
· Licensed Independent Clinical Social Worker (LICSW)
· Licensed Graduate Social Worker (LGSW)
· Physician (M.D. or Osteopathic Doctor)
· Nurse

To whom it may concern,
______________ presented to our practice with neck and back pain. She has been following our treatment plan with moderate results, including decreased pain and increased range of motion. Due to her continued muscle hypertonicity, and decreased cervical range of motion, it is my clinical opinion that __________ would further benefit from massage therapy as a co-treatment, in her treatment plan.
To whom it may concern,
_______________ presented to my practice with symptoms related to her trauma history. We have been working together and she is making gradual progress in the process of trauma healing. Due to her continued symptoms, and the potential for your modalities to speed her progress, I am referring her to you for bodywork as a co-treatment, in her integrative treatment plan.
